Typical configuration
- Identity fields: just Location, Photo, Description. Optional severity defaults to Low.
- Effect: severity medium or high inserts a CAPA record and links back.
- Cron: every Monday 08:00 tenant-time — digest email to safety leaders with NM count, trending location, and top-3 themes.
- Permissions: Anonymous writes allowed (no session), but reveal of reporter requires the Safety-manager role.
