How it works

  1. Capture the incident

    Open the QEHS mobile app, tap "New incident", enter a description, add a photo, and confirm the GPS location.

  2. Classify severity

    The guard tree asks four questions and auto-classifies the incident as near-miss, first-aid, recordable, or lost-time.

  3. Assign investigator

    The workflow automatically routes the record to the designated investigator for the affected location.

  4. Complete root cause analysis

    The investigator works through a five-whys or fishbone analysis, attaches evidence, and sets corrective actions.

  5. Close and report

    Once effectiveness checks pass, the record closes, the OSHA 300 log updates, and a digest lands in the safety manager's inbox.
